How It Works: One Audit Request, Start to Recorded

This page follows a single evidence request through the loop — from the moment it arrives to the moment the answer is on the record. The request comes from an auditor, but the same stages serve a regulator, an insurer, or your own leadership asking the same question.

Stage 1

The request arrives: three questions, precisely stated

The request is specific, as audit requests are: produce the information-security policy as it was in force on March 1; produce the acknowledgment records for that version; and produce the current business license, with its status. In a folder-based world, this is where the scramble begins — which copy was in force in March, and who kept the acknowledgment list?

In the loop, the request itself is captured as an item with an owner and a due date. Nothing about it lives in one person's inbox.

Stage 2

Evidence is located in the repository

The Outcome Repository™ holds the policy with its full version history, so "the version in force on March 1" is a direct lookup: the version effective on that date, the one it superseded, and the one that followed. The acknowledgment records sit against that exact version — names, dates, and signatures — because they were captured when the version took effect, not gathered afterward.

The license is retrieved the same way, with its expiration date and renewal history attached. Every retrieval in this stage is logged: what was pulled, by whom, and when.

Stage 3

The record is assembled — nothing recreated

The answer takes shape from the record itself: the policy version, its acknowledgment list, the certificate and its status, each with its history. Nothing is retyped, re-dated, or reconstructed from someone's memory of what the policy probably said in March. Where the record shows a change — a version replaced mid-period, an acknowledgment completed late — the history shows that too, plainly.

That plainness is the point. An answer assembled from a living record can be defended line by line, because every line points back to an entry that was made at the time.

Stage 4

One gap is found — and resolved

The record is honest, so it shows a gap: one site never completed acknowledgment of the March version — three people, still open. In a folder-based process this surfaces as an audit finding weeks later. Here it surfaces now, as a resolution item in Outcome Workspace™: the item names the version, the people, and the owner responsible for closing it.

The acknowledgment is routed, signed, and completed, and the closure is written back to the record — who closed it, when, and how. The gap is not hidden from the answer; it is answered, with its resolution attached.

Stage 5

The answer is recorded as an outcome

The response goes out: the version, the records, the certificate, and the resolved gap. Then the loop closes the item and writes the outcome to the record — what was asked, what was produced, what was found, and what got done with it. The next request starts from a record that is stronger than the one this request found.

That record is what Outcome Analytics™ reports on: requests answered, acknowledgment completion, gaps found and closed, certificates by time-to-expiry. Readiness stops being a claim and becomes a number the team can watch.

Requests arrive through a front door, too

Not every request arrives by letter or email. An evidence request submitted through a form or portal — MaxxLaunch™ on the MaxxLogix platform — is captured as structured data at intake and enters the same loop as an owned item. However the question arrives, the answer comes from the same record.

The same discipline, every document

Policies, certificates, licenses, evidence files — each is versioned, acknowledged where it must be, watched where it expires, and retrieved when it is asked for. The stages do not change from document to document; that consistency is what makes the record trustworthy as a whole.

Answer the next request from the record

The stages above run on MaxxLogix — Outcome Cloud™, the Outcome Repository™, Outcome Workspace™, and the rest of the platform, with capacity published in SI Units and no surprise overages. See it at maxxlogix.com.
